Registration Form
(Use this form to register for WDSRA programs via fax, regular mail or in person)
2008-2009 Annual Form
(All participants are required to fill out an annual form each summer which is good through the following Spring)
Special Olympics Medical Form (THIS FORM MUST BE MAILED IN. FAXES WILL NOT BE ACCEPTED)
(Special Olympics athletes must have a medical form on file to be eligible for Special Olympics competition)
Accident/Illness Form
(This form is for any participant in summer camp or programs that involve an overnight stay)
Scholarship Form
(This form is for WDSRA participants requesting financial assistance for programs)
Miracle Field Application For Use
(This form is for groups wishing to use the Miracle Field for practices or games. Fax or mail to the address on the form.)
